The dough never moved, or moved and then sank, and the loaf came out flat and heavy. The cause is almost always one of seven things: dead yeast, water too hot, salt in the wrong place, a cold kitchen, too little kneading, or a prove that went too short or too long. All of them are easy to fix, and most are easy to diagnose before you even get to the oven.
First, work out what your dough did
The dough's behaviour tells you which cause to suspect, so read it before you change anything.
- It never grew at all. After a couple of hours the bowl looks exactly as it did when you put the dough in. That points at the yeast: dead, never activated, or killed at the mixing stage.
- It grew slowly, then stalled. The dough half-doubled and stopped. That's usually temperature: a kitchen too cold for the yeast to work at full pace.
- It doubled, then collapsed. The dough rose nicely and then sank back, and the surface may look wrinkled or the dough smells beery. That's over-proving: the gluten structure gave out.
- It doubled but the baked loaf is still dense. The rise happened and then the gas escaped, which points at kneading, or at a second prove that never finished.
The yeast was dead (the most common cause)
Yeast is a living organism, and baking bread is a small act of husbandry. Fast-action dried yeast has a shelf life, and a sachet past its date, or one that has been sitting open in a warm cupboard, is dead. The easiest way to check is to test it before you commit: dissolve a teaspoon in warm water with a pinch of sugar and wait ten minutes. Alive yeast produces a frothy layer; dead yeast does nothing.
There is a second way to kill it, and it's the one beginners hit most. The water. The bread course calls for water at roughly body temperature, about 37°C, and the reason is precise: too cold and the yeast won't activate, too hot and it dies on contact. If you don't have a thermometer, mix one part boiling water with two parts cold from the tap. It feels like warm bathwater, not hot.
The salt was sitting on the yeast
Salt controls yeast, and in direct contact it kills it. The standard beginner recipe has you weigh 500g strong white flour, then add 7g fast-action yeast and 10g salt to opposite sides of the bowl, and there is a reason for that arrangement. If the salt and yeast touch while the flour is dry, the yeast cells die before the water ever reaches them. A pinch of salt dissolved in the water is harmless; a heap of salt sitting directly on the yeast granules is a funeral.
The kitchen was too cold
Yeast works on a schedule set by temperature. The classic 1–2 hour prove assumes a normal room, roughly 20–24°C. In a cold kitchen, or in winter with the heating off, the same dough can take three or four hours to double, and many people bake it before it ever gets there. The fix is not a warmer water mix (that ship has sailed once the dough is made) but a warmer place to prove: a radiator shelf, an airing cupboard, or the oven with just the light on and the door shut. Judge by the dough, not the clock: it should double in size, spring back slowly when you press a finger in, and look puffed rather than slack.
The dough wasn't kneaded enough
Kneading does two jobs: it develops the gluten network that gives bread its structure, and it distributes the yeast so it can feed evenly. The dough should be smooth, elastic and slightly tacky after eight to ten minutes of hand kneading. If it's rough, torn-looking, or still sticky at the end of your kneading session, the gluten hasn't developed, and what happens next is a loaf that rises and then lets all the gas escape, giving you a dense, close crumb that never had a chance.
The test is the windowpane: pull a small piece of dough and stretch it slowly. If it stretches thin enough to see light through without tearing, the gluten is there. If it tears at the first pull, keep kneading. Two minutes of extra kneading fixes what a flat loaf cannot.
The prove went wrong in either direction
Under-proving is simple: the dough went into the oven before the yeast had done its job, so the loaf bakes small and dense. Over-proving is the more interesting failure. Leave dough too long and the gluten network, stretched past its limit, starts to break down. The dough that doubled at hour two is collapsed and wrinkly at hour five, and the loaf that results is flat with an open, coarse crumb and a slightly sour smell. If your dough doubled and then sank, this is your cause, and the fix next time is a timer, or a prove at a slightly cooler spot so the window is wider.
The flour let you down
Finally, check the bag. Bread needs strong flour, which carries more gluten-forming protein than plain. Make a loaf with plain flour and you get something more like a damp scone: it rises, then can't hold the shape. It's not a failure of technique, it's a mismatch of ingredients. Strong white flour, £1 or so a bag, removes the problem entirely.
Fixing this loaf and the next
If the dough is still in the bowl, you can often save it. Test the yeast; if it's dead, knead a fresh sachet in, keeping it away from the salt. If it's alive, give the dough warmth and time. A dough that has collapsed from over-proving is the one case that usually can't be rescued: it will re-rise weakly and bake dense. Cut your losses, start again, and use the checklist below.

Common questions
How do I test if my yeast is still alive?
Dissolve a teaspoon of the yeast in warm water with a little sugar and wait ten minutes. A frothy layer on top means it's alive; a flat surface means it's dead. An expired sachet of fast-action yeast is the most common culprit in a loaf that never moves.
Can I rescue dough that hasn't risen?
Often, yes. Test the yeast first: if it's alive and the dough is just cold, move the bowl somewhere warm and give it longer. If the yeast is dead, knead a fresh sachet into the dough, being careful to keep salt away from it, and start the prove again.
Why is my bread dense even though it rose?

Does the temperature of my kitchen matter?

What's the difference between strong flour and plain flour for bread?
